Please, download Order Form of Land or Air program
and Medical Form, print them, fill in, and post them to us with original
signatures and doctor's stamp, and also 4 photos and copy of Passport
(all filled pages).
To speed up the process of preparation of the program you may fax us Order Form (380-44-517-51-88) or send by e-mail (tour@info.kiev.ua). However, final acceptance and confirmation will only become effective upon receipt of the ORIGINAL of those Forms with photos.
Then we will send you the final confirmation with schedule of the program, price, and conditions of payment.

Conditions of participation:
- You must be over 21 years of age.
- You should be in acceptable health and mentally prepared to participation in program.
- You must hold a current (valid) Passport.
